[Treatment of superficial esophageal cancer by argon plasma coagulation].

چکیده

アルゴンプラズマ凝固法(argon plasma coagulation: APC)は外科手術時の臓器切断面止血法のひとつ として用いられてきたが,内視鏡用のアプリケータが 開発されてからは,消化器内視鏡分野で新たな止血 ・凝固法として注目を集めている. APCはアルゴン ガスを噴射し,その流れに高周波電流を重ねた非接 触型高周波凝固法で,アルゴンガスはプローブ先端 でイオン化されアルゴンプラズマ流となって組織に 到達し,表面を凝固する.その適応は止血処置の他, 腫瘍焼灼や食道静脈瘤の地固め療法にも使用され ている.特徴としては,操作が簡便で短時間に広範 囲を浅い深度で凝固できることがあげられる.今回, 重篤な基礎疾患を合併しているため内視鏡的粘膜切除 (Endoscopic mucosal resection: EMR),外科手術が 施行困難な食道表在癌に対しAPC治療を施行した 症例を提示し,その有用性を報告する.
